How to edit COVID consent form and request signature

** You will need to have owner access for this. **

STEP 1: Click 3 vertical dots

STEP 2: Click on Salon Settings

STEP 3: Enter Password

STEP 4: Click Configuration

STEP 5: Click on the APP tab

STEP 6: Expand the CONSENT FORM section

STEP 7: To add or remove the COVID consent form pop-up

NOTE : Black radio button means not select (will not pop-up) and blue radio button means selected (will pop-up)

STEP 8: To request signature for COVID consent form, make sure "Request Sign" radio button is selected.

STEP 9: To edit the message for the COVID consent form, use the text box

STEP 10: When you are done with your edits, click on SAVE.
